To be eligible, applicant must:

be 21 to 70 years of age

have contact phone number (landline) in both home and business address

 

DOCUMENTS REQUIRED:

 

For Employed, submit any ONE of the following documents:

Photocopy of the latest ITR

Photocopy of last 2 months Credit Card Statement (of the same credit card)

Original Pay Slip for 1 month (with company name and/or employee ID/employee number)

Original Certificate of Employment (COE)

 

For Self-employed, submit any ONE of the following documents:

Photocopy of the latest ITR

Photocopy of last 2 months Credit Card Statement (of the same credit card)

Photocopy of Certificate of Business Registration together with a photocopy of Audited Financial Statements

Photocopy of last three months Bank Statement and Original Bank Certificate

 

ADDITIONAL REQUIREMENTS:

 

For both employed and self-employed applicants, attach a clear and reliable photocopy of any ONE of the following:

Passport

Driver s License

Firearms License/PTC

SSS/GSIS ID

NBI Clearance

BIR/TIN ID

Senior Citizen s ID

IBP/PRC ID

Major Active Credit Card (with photo)

ID of top 10,000 Companies (with photo and signature)
